About Us

At You Matter Primary Care, we believe healthcare should feel personal, not rushed. Founded by Brenda Herrera, FNP-BC, a board-certified Family Nurse Practitioner. She is passionate about providing thoughtful, individualized, and thorough care and believes patients deserve time and attention at every visit. Every visit is scheduled for a full hour, allowing us to listen carefully, understand your concerns, and create thoughtful, individualized care plans. We care for adults 18 and older, focusing on prevention, chronic disease management, and whole-person wellness.

Originally from Peru, Brenda is a native Spanish speaker and is deeply committed to serving diverse communities with culturally competent care. Her approach is rooted in listening, education, and empowering patients to take control of their health.  Our mission is to make healthcare accessible, respectful, and empowering-especially for individuals and families from diverse backgrounds

This fellowship was created to bridge the gap between nurse practitioner education and real-world primary care practice to build confidence before independent practice.

This is a 4-week, small-cohort fellowship (2 participants/month) focused on clinical reasoning, workflow, and real patient care-not rushed clinical rotations.

Why this Fellowship is Different

Unlike traditional clinical experiences:

  • Longer patient visits designed for deeper learning and clinical understanding

  • Direct Mentorship in a real NP-owned private practice

  • Focus on how to think, not just what to do

  • Exposure to real workflows (insurance, pharmacy, documentation)

  • Exposure with a diverse, predominately Spanish speaking population

What You'll Gain

By the end of this fellowship you will feel more confident in:

  • Clinical decision making in primary care

  • Interpreting labs and imaging

  • Managing chronic disease (Diabetes Type  2, Hypertension, Hyperlipidemia, Obesity, etc.)

  • Writing medical documentation (work notes, FMLA, letters)

  • Navigating real-world practice challenges 

  • Career readiness and Job Selection

  • Billing and coding 

Fellowship Structure

  • Duration: 4 weeks

  • Schedule: 3 clinic days per week

About Your Instructor 

This fellowship is led by an experienced Family Nurse Practitioner with:

  • Over 10 years in primary card 

  • Private practice ownership 

  • Extensive experience precepting NP students

  • A strong focus on patient-centered culturally competent care

TYPES OF APPOINTMENTS OFFERED

We provide: Annual physical exams, chronic disease management (Diabetes, hypertension, etc.), mental health management, sick visits, weight management, and women's health services.
​Televisits are also available for mental health, acute care appointments, and in for follow up visits.

PREVENTATIVE CARE

60-120 minutes

Annual physicals include being ordered fasting labs, screenings, vaccines, and more​ based on your individualized risks.

​​​- Receive detailed, thorough, and personalized annual physicals including cancer screenings, electrocardiograms, fasting labs, and vaccinations. 

​​​

- Fasting labs are individualized based on your risks however they will most likely consist of a fasting cholesterol panel, fasting glucose included in a complete metabolic panel which checks on your kidney, liver, and electrolytes, complete blood count which checks on your platelets , immune system cells, and hemoglobin, thyroid stimulating hormone, urinalysis if needed, other labs can include vitamin D, vitamin b12, prostate specific antigen for men, and cardiovascular risk markers like your lipoprotein A level and apolipoprotein B.

- Screenings are based on your age and your individual risk factors. 

- These include pap smears and pelvic exams as well as male genitourinary exams including digital rectal exams if needed.

- Receive evaluation for and management of men's health services such as erectile dysfunction.​​

- Receive contraception management if needed

- Ear irrigation may be provided if needed.

- Physicals forms are filled for insurance or employers if needed.

​​​

- Annual physicals are in-office only.

CHRONIC DISEASE MANAGEMENT

60-90 minutes 

Diabetes Mellitus Type 2, High Blood Pressure, High Cholesterol, Asthma, Allergies, Fatty liver, COPD, Sleep apnea, Hypothyroidism, and many more

- Receive dedicated and thorough maintenance follow ups every 3 to 6 months depending on the number of chronic diseases and severity of disease.


- Let's work together on an individualized treatment plan you understand and agree with. 


- Receive empowerment through education on the disease with a focus on lifestyle changes and preventative care 


- May including fasting labs done 3 days before your visit to review your results during appointment in detail.


- Depending on the number of diseases, extra time will be allotted, they can be from 60 to 90 minute visits.

- FMLA and other work forms are filled if needed.


- Coordination of care is important for effective and thorough chronic disease management with various specialists such as seeing a cardiologist for a detailed cardiovascular risk evaluation.

- Telemedicine available in case by case basis as in-office visits are needed to get updated vitals and physical exam.  

MENTAL HEALTH SERVICES

60-90 minutes

Diagnosis and management of Anxiety, Depression, PTSD, Eating disorders, Insomnia, ADD/ADHD, and Grief

- You will be heard and supported during our time working together on improving your mental health and building an individualized treatment plan you feel comfortable and confident with.

- Mental health is crucial for your overall health.

​​

- Telemedicine available.

- You will be referred as well to a therapist or counselor and if needed a psychiatrist, Psych NP, or psychopharmacologist for the initial diagnosis of ADHD, and diagnosis and management of other mental health disorders such as Bipolar Disorder.

ACUTE CARE

30-60 minutes 

Upper and lower respiratory infections including sinusitis and bronchitis, Urinary tract infections, COVID-19, Strep, Flu, Abdominal pain, Rash, Vaginal infections, and many more

- Receive evaluation and management of sudden acute symptoms such as new respiratory symptoms like a cough or gastrointestinal symptoms like abdominal pain.


- You may get stat labs, COVID-19 tests, flu test, strep test, finger glucose test, finger hemoglobin test, urinalysis, electrocardiogram, nebulizer treatments, ear lavage, and/or pregnancy test if needed based on your symptoms.

- Telemedicine available.

PRE-OPERATIVE CLEARANCE

60-90 minutes

Need clearance for surgery? We are here to help!

- If you are going to have surgery, your surgeon may require a medical clearance by your primary care provider that may include labs, an electrocardiogram, and sometimes a chest x-ray.


- Your pre-operative in-office only visit will include the above and will be individualized based on the medications you take and your risk factors.

- Pre-operative visits are for established patients only.

- If you have increased cardiovascular risks, you will be referred to a cardiologist to always be on the safe side for a more thorough cardiac evaluation and clearance.

TRANSITIONAL CARE MANAGEMENT

60-120 minutes

If you have recently been to the emergency room or discharged from a hospital, rehab, or skilled nursing facility, we are here to help you transition back home and assure continuity of care

- Receive attentive and detailed medical care and emotional support that is needed after a hospital discharge.

​​

- We will help you understand your hospital records that should include any testing done while you were there such as labs and imaging and the treatment plan which can include new medications and referrals needed to other specialists. 

​​

- Telemedicine available.

- Throughout this process we will assure we are always considering your health goals and focus on wellness and prevention.

- We will work together on the next steps in your care such as helping make specialists appointments if needed along with following up on any abnormal hospital findings.​

Our Pricing

INSURANCE OPTIONS

We now accept the following insurances: Aetna, Anthem BCBS, Carefirst, Tricare, Cigna, and United Healthcare

Please call your insurance to assure we participate with your specific plan. We no longer participate with Anthem Healthkeepers plus or other Medicaid based insurances.

We ask for payment at the time of service however we do allow payment plans and offer patient memberships. We will provide you with your itemized medical bill along with your proof of payment for you to submit to your medical insurance to seek reimbursement except for patients who have Medicare as they will not reimburse for services provided.

Our Memberships

We offer Monthly and Yearly Memberships for patients with no insurance or out of network insurances:

 

Our Memberships offer:

- Unlimited appointments which includes your yearly physical, sick visits, telehealth visits, and follow ups with no additional costs per visit.

- Free in-house diagnostic testing to include electrocardiograms (EKG), urine dipsticks, pregnancy tests, peak flows, albuterol nebulizer treatments, wound cleanings, wart freezing except for facial warts, pelvic exams, COVID-19/Strep/Influenza tests, diabetic monofilament tests, vision, and hearing screenings.

- Reduced lab prices through Quest labs.

- There is a three month minimum for monthly memberships then can be cancelled when desired.

- Please call the office for more information during our operating business hours.
